Is it possible to use an iPhone 4 as modem for my MacBook Pro?

Technically, the answer is no by definition of the word "modem".
However, if you're really asking if you can get an Internet connection on your MacBook Pro through your iPhone, the answer is yes. In order to do this, you will need to enable the Personal Hotspot feature and be on a data plan from your wireless provider which supports this feature. A personal hotspot creates a Wi-Fi network which your MacBook Pro can then connect to like any other Wi-Fi network.

Technically yes, but it's a feature that needs to be supported by the carrier. In other words you'll likely have to pay to unlock this capability. I would inquire with your carrier.

That might depend on what carrier you are with - the term your looking for is Personal Hotspot this can be found in settings.
Pay as You Go - Doesnt support Personal Hotspot - the button in settings will not be visible
Contract - again if you have a data plan with the contract this wont determine a personal hotspot feature
contact the carrier for plans because this is sometimes an additional charge
