The length of an HDMI cable does have a negative effect on signal quality, with manufacturers generally not recommending cables over 20 feet (6 meters).
To cover more distance, look into HDMI Balun Kits, which convert to ethernet and back. But it is probably easiest to keep your HDMI devices close together.

Not to simply mirror the TV output and sound from your device to the TV through HDMI. The second lightening female adapter permits charging your device while using it. But the charging is optional. In other words it works the same as the Apple Lightening to HDMI Only adapter but with an added advantage of charging in case you are streaming for a long time.

There shouldn't be. It's just a smaller version of the same thing. If you have an iPhone 6S or later then you should be able to output at 1080p (full HD but not 4K). Maybe play around with the resolution settings of the actual video you're trying to watch - for example, youtube videos have a setting where you can adjust the output quality of the video, up to the resolution the video itself was actually uploaded in.

I just purchased this adapter to use with my iPhone 8 and a monitor that does not have speakers or audio capability. I am unable to play audio through the phone’s integrated speakers when the adapter and HDMI cable to the monitor are connected to the phone. The HDMI cable appears to override any options you may have to direct audio to any output other than the HDMI device at the other end of the cable. If the HDMI cable is connected to a TV or other device that allows both video and audio input, then it should work. However, if you don’t have such a device, you may have the same problem with hearing audio when the adapter and an HDMI cable are connected.

It won't charge directly from the TV. The Lightning Digital AV Adapter does have an extra port to plug in your iPhone or iPad charger and therefore you can charge and watch TV at the same time.
