For your convenience, Apple is loaning you an iPhone with serial number ({SERIAL}) while your iPhone is being serviced, subject to the iPhone Loan Agreement referenced below. Pursuant to its terms:
- You have 14 days to return the loaner phone and pick up your iPhone once Apple notifies you that your iPhone is ready.
- You may be held responsible for repair or replacement costs if the Loaner Phone is lost, damaged, or stolen while on loan.
By signing this Genius Bar Work Authorization, you acknowledge that you have reviewed, agreed to, and received a physical copy of, the iPhone Loan Agreement, containing the full terms and conditions governing this loan. A copy of the iPhone Loan Agreement may be found here
